What: Assessment and recommendation of professional and personal care services based on client and family needs and community services available.
When: Mon-Fri, 8 a.m. – 6 p.m., Sat-Sun, 8 a.m. – 4:15 p.m.
How: Call 403-943-1920, complete and fax Integrated Home Care Physician Referral form to 403-943-1602. For other Healthcare Providers, complete and fax Community Care Access Referral form to 403-943-1602. Include: Past medical hx, medication list, allergies, diagnoses, reason for referral, goals of care designation.
Response time: Varies based on requested services and need.
Billing code: Advice 03.01 NG, 03.03 NH, 03.01 NI, Formal Conference 03.05 JA (included on Provider Report)
Documentation: Primary Care Provider report is faxed. Includes admission status, goals and services provided.
